Patients who are due to come into your practice for NHS treatment can read and sign the NHS Patient Declaration form online with Dentally's NHS form available through the Dentally Patient Portal. This saves you time and paper. It keeps the electronic forms safe and further promotes a contactless reception.

On some occasions, the patient may not be able to sign the form for certain scenarios and reasons. In order to help educate the patient on why this may not be available, we have added the reason at the top of the form page. This will look like the following example:

Below are the scenarios when a patient is unable to sign the form:

The patient doesn't have an upcoming appointment

The patient already has a signed NHS PR for the course of treatment which is active in their chart tab

The upcoming appointment is not with a dentist (or administrator)

The upcoming appointment is greater than 14 days from todays date

As a practice staff user, can I see this information?

You can check on this by impersonating the patient from the Dentally Portal Manage app from manage.dentr.net and clicking View Portal then select As an existing patient. Search for the patient you wish to check and in the preview screen on the right, click on the menu tile labelled as View your NHS details.
