Can patients complete their NHS forms through Portal?

Patients who need to complete an NHS patient declaration in advance of their appointment with you can do so digitally. This makes it a smoother process for you and your patients, reduces waste and improves data protection.

If you are a Dentally Portal user, patients can complete an NHS form using Portal. There are two NHS forms supported by Dentally Portal:

๐Ÿšจ Important: The GP17 option for Portal is currently in Beta testing, and is not yet widely available. If you can't see it yet, you will do soon.

Patients can complete these forms in a number of ways, depending on which form they need, and what features you have enabled:

  • If you have task reminders enabled, patients can complete an NHS form by clicking a link in a reminder text (FP17 only).

  • Patients can log into the patient portal on their own device to complete a form (FP17 only).

  • If you have Concierge enabled, patients can use a tablet device on arrival in the practice to complete their NHS form (both FP17 and GP17).

๐Ÿ’ก Top tip: If you are using the 'Visits' tool to manage patient arrivals/actions in your practice, the panel will also indicate whether the patient needs to complete a GP17 form.

When an NHS form has been signed digitally, it will automatically go through to their patient record. It can be viewed, along with any exemptions, in the 'NHS' tab in the patient's record.
