When are my patients required to complete a GP17 form?

Patients will be asked to complete Part 1 of a GP17 form if:

  • They are an NHS patient

  • They have an appointment today

  • Their appointment is not linked to an existing treatment plan appointment

๐Ÿšจ Important: The GP17 option for Portal is currently in Beta testing, and is not yet widely available. If you can't see it yet, you will do soon.

If these criteria are met, Part 1 of the GP17 form is automatically generated for patients to complete using Portal.

They will be asked to complete Part 2 of a GP17 form once their course of treatment has been marked as complete in Dentally - in this instance, Part 2 of the form will be automatically generated.

How can my patients complete Part 1 of their GP17 form digitally?

If a patient needs to complete a GP17 and meets all the requirements, they will be prompted to complete the first part of their GP17 upon arrival in the practice for their appointment.

๐Ÿ’ก Top tip: GP17 forms can only be completed digitally on a Concierge device in the practice.

To complete Part 1 of the GP17:

  • When the patient arrives in the practice, hand over your Concierge-enabled device and instruct them to sign in with their date of birth and surname initial.

  • They should select 'Fill in your NHS form' from the outstanding task list on their home page.

  • They will be prompted to select an option about where they are registered, and click 'Continue'.

  • They can then select an appropriate exemption, and click 'Confirm'. Note that certain selections, such as 'Income Support', will prompt additional fields such as National Insurance Number, which need to be completed before proceeding.

  • Once they have selected and completed the required fields, they must confirm they agree by providing their signature and clicking 'Sign'.

  • If they are signing on behalf of another patient, they must tick the box to indicate this. They will then be prompted to provide their name and their relationship to the patient.

  • Once they have clicked 'Sign', they will be taken back to their dashboard and will see the 'Fill in your NHS form' action card is now ticked and crossed out.

The patient will hand the tablet back once they have finished their actions, and then will go in for their appointment. If, at the end of their appointment, the dentist marks their course of treatment as complete, Part 2 of the GP17 form will be generated for them.

How can my patients complete Part 2 of their GP17 form digitally?

If a patient needs to complete a GP17 and meets all the requirements, and has completed Part 1 of the GP17 and attended their appointment, they will be prompted to complete the second part of their GP17 once the practitioner marks their treatment as complete.

To complete Part 2 of the GP17:

  • Following the patient's appointment, someone from your team can hand over your Concierge-enabled device again and instruct them to sign in with their date of birth and surname initial.

  • They should select 'Fill in your NHS form' from the outstanding task list on their home page, which should reappear in the list following their appointment.

  • Depending on what they selected in Part 1 of their GP17, they may be prompted to select an appropriate exemption if applicable, and click 'Confirm'. Note that certain selections, such as 'Income Support', will prompt additional fields such as National Insurance Number, which need to be completed before proceeding.

  • They will then be shown the outstanding balance due, if applicable - if they do not need to pay anything, this will be indicated here.

  • They can then click 'Agree & sign', and confirm they agree by providing their signature and clicking 'Sign'.

  • If they are signing on behalf of another patient, they must tick the box to indicate this. They will then be prompted to provide their name and their relationship to the patient.

  • Once they have clicked 'Sign', they will be taken back to their dashboard and will see the 'Fill in your NHS form' action card is now ticked and crossed out.

Once the patient has completed one or both parts of the GP17 form, you will be able to view them in the patient's record, in the NHS tab (under 'Exemptions')
