When are my patients required to complete an FP17 form?
A patient will be asked to complete an FP17 form if:
They are on the NHS payment plan in Dentally
They have an appointment booked with a dentist
Their appointment is within the next 2 weeks
They do not have an existing open treatment plan with NHS UDA values assigned
An FP17 form has not already been signed within 14 days of their upcoming appointment
If these criteria are met, the FP17 form is automatically generated for patients to complete using Portal.
How can my patients complete an FP17 form digitally?
Patients will be prompted to complete their FP17 up to 14 days before their next dentist appointment.
If a patient needs to complete an FP17 and meets all the requirements, they will be prompted in the patient Portal to complete the form.
๐กTop tip: If your practice has Concierge enabled, patients will be able to follow the below steps on a tablet device in the practice as well, in addition to being able to complete the form at home in advance.
They then select an appropriate exemption, or select 'I am not entitled to any exemptions', and click 'Confirm'.
They will be prompted to enter their HC2 certificate number and click 'Confirm'.
Once they have completed all required fields, they can confirm the data is correct and proceed.
They should click 'Agree & sign', then confirm that they agree by providing their signature and clicking 'Sign'.
If they are signing on behalf of another patient, they must tick the box to indicate this. They will then be prompted to provide their name and their relationship to the patient.
Once they have clicked 'Sign', they will be taken back to their dashboard and will see that the 'Fill in your NHS form' action card is now ticked and crossed out.
Once the patient has completed the FP17 form, you will be able to view it in the patient's record, in the 'NHS' tab (under 'Exemptions')
