How do I send a medical history form to a patient via SMS?
๐ Please note: Previous medical history entries for the patient will remain in their record, and won't be overwritten by the new one.
You can send patients a link that is sent is entirely secure and unique to each of them. The link is single-use, the patient cannot use it more than once (even if they didn't actually complete the form the first time they clicked it) - if it stops working, they will need to request a new one.
Completed medical history can be viewed and managed from the patient record.
To ask the patient to update their medical history via SMS link:
In the patient record, go to the 'Medical' tab.
Click the down arrow โฌ๏ธ in the right-hand corner. You will see a dropdown menu appear.
Click 'Send via SMS...'.
In the window that appears, you may customise the message you are sending the patient. There is a limit of 115 characters.
When you are ready, click 'Send'. The patient will then receive a link via SMS, which they can use to provide updated medical history.
When the patient has completed their medical history using the link, it will automatically be added to the patient record.
๐ก Top tip: By default the medical history questionnaire is pre-populated with a patient's previous medical history details. This prevents them from having to type in all their data every time.
Alternatively, if you are a Portal/Online Signing user, you can ask the patient to update their medical history through Portal.
Links can be sent manually like this, or the practice can use the data tag [patient_medical_link]
in any correspondence template, which will automatically populate with a link unique to the recipient. This allows you to send via email as well as SMS.
