How to edit your practice's medical history questions

How do I edit my practice's medical history questions?

  • Navigate to your settings and click 'Treatments & Plans'.

  • Select the 'Medical History Questions' tab.

  • To edit a question, click on the text of the question - it will now open as a text box and allow you to type. You can do the same for 'Follow up if answered Yes' questions.

  • To save, simply click the grey area outside the text box to close it. This will record the changes you have made.

  • To delete a question, click the bin ๐Ÿ—‘๏ธ icon next to it.

  • To add a new question, click 'Add medical history question' at the bottom of the page. This will add a new question to the bottom of the list - click to edit it as before.

  • Click 'Add follow up question' to add a follow-up to any existing question, which the patient will only see if they answer yes to that question. This defaults to 'Please provide details' but can be edited by clicking on it.

  • To change the order of your questions, hover over any question and you'll see the three line icon - click this and drag up or down to move.

  • You can set how often you wish patients' medical history responses to be marked as 'Out of date', meaning a prompt for a new one will be generated. At the top of the page, you will see 'Warn after X days'. Edit this number to change the interval. Click the grey area outside the text box to close it. This will record the changes you have made.
