How do I view a patient's medical history?

To view a patient's medical history:

  • In the patient record, go to the 'Medical' tab.

  • From here, you will see a list of all medical history forms that the patient has ever completed.

  • The entries will state when it was created, who by and other added information. If the patient completed their medical history using Portal, this will be indicated by a small tablet ๐Ÿ“ฑ icon.

  • The heart โ™ฅ๏ธ icon indicates any cause for concern. If the heart icon is green, there are no issues flagged by the medical history entry (eg all answers were 'No'). If the heart icon is red, the patient has flagged a medical issue which may require attention. You can also choose to add medical alert for the patient, which flags the chosen medical information at the top of the patient record next to their name.

To view a particular medical history, find the one you need in the list and click the 'View' button on the far right of that entry. While viewing a medical history entry, the actions you can complete are:

  • 'Edit' - Only visible if the medical history was created less than 24 hours ago. You can use this to edit the patient's answers or add/remove notes. After 24 hours the entry is locked and this option will no longer be available.

  • 'Delete' - Only visible if the medical history was created less than 24 hours ago. You can delete the entry entirely, if it is incorrect. If deleted, a flag will be added to the patient's record confirming that an updated medical history is required. After 24 hours the entry is locked and this option will no longer be available.

  • 'Reviewed' - This button is visible under the 'Mark as' dropdown button. Use this to add a marker confirming that the entry has been reviewed by your team (for example, you may review it to check it has been completed correctly).

  • 'Invalid' - This button is visible under the 'Mark as' dropdown button. As it cannot be deleted or edited if it is locked, you may wish to mark it as 'Invalid' if the patient has informed you that it is inaccurate. You will be asked to enter a reason for the invalidation, and a marker will be added. Once selected, this cannot be undone. A flag will also be added to the patient record stating that a new medical history is required.

  • 'Print' - You can print a physical copy of the medical history entry if you or the patient require it.

There are three ways to update a patient's medical history.


How do I update a patient's medical history manually?

You can manually update a patient's medical history (for example, if they have completed a physical form which you need to input) - please note that previous medical history entries for the patient will remain in their record, and won't be overwritten by the new one.

  • In the patient record, go to the 'Medical' tab.

  • Click the 'New medical history' button in the top right corner (the button itself, not the down arrow to the right of it).

  • The questions will be auto-populated with the patient's previous answers, if they have completed medical history before.

  • Click each question to toggle the answer between 'Yes' (red) and 'No' (green).

  • There are boxes at the bottom to add notes from the patient or from yourself.

  • When you have finished, ensure you click 'Save'.

๐Ÿ’ก Top tip: Once you have submitted a medical history, you can edit or delete it for up to 24 hours - then it will be locked. If you need to update their medical history after that point, you will need to record a new medical history.
