How to complete a Case Mix assessment for a patient?

Where to fill out a Case Mix form for a patient and how to report on your past Case Mix assessments, for NHS practices.

How to complete a Case Mix form for an NHS patient?

You must already have an NHS treatment plan open before being able to fill out an NHS Case Mix form.

  • Start by charting an item onto your open treatment plan as you normally would.

  • The patient must be set up with the NHS payment plan.

  • You will then see a green pop up appear prompting you to complete a Case Mix form. Click on the 'Show Case Mix' button on the right hand side.

  • Fill out the six case mix prompts as per the guidance from the BDA and add any comments needed before being clicking 'Save'.

The Case Mix answers can be viewed again from the same button as long as the treatment plan is still open.

How to View past Case Mix forms?

To view Case Mix assessments for past completed treatment plans you can use the 'Case Mixes' report from the main 'Reports' menu.

Add your 'From' and 'Till' date filters, to view all case mixes filled out in this time frame.

You can also use the blue hyperlinked 'Patient' or 'Treatment plan' number to jump to the treatment plan in question.
