How do I enable and disable online forms?

  • Go to 'Online Signing' in the left-hand toolbar, and click 'Document types'.

  • You will see all the forms available for your practice, and you can turn them on and off.

๐Ÿ“ Please note: Availability of some forms, such as NHS PR forms, will vary depending on your region. The medical history and treatment plan estimate forms are global.

๐Ÿ’ก Top tip: If you enable the 'Medical histories' form, patients can complete the form on Portal with a secure two-factor authentication login, and the form will be pre-filled with previous answers (if they have completed it before). If you disable this form, patients can still complete it online using merge tags as normal, but the above Portal features won't be available to them.
