How to create an estimate/consent form for a patient

How do I generate an estimate/consent form for a patient?

With a treatment plan in place, you can produce an estimate for the patient to review and sign with a couple of clicks.

  • Once the proposed treatment plan is charted click the ‘Create Estimate’ button.

  • If you are an Australian practice you will have the option here to 'Create a health fund estimate' instead.

  • Select the template you wish to work from from the 'Templates' drop down.

  • You can then make edits to this template as needed for the specific patient.

  • Once you are happy with the content, click the 'Create' button.

  • This will open your form as a PDF where you can print, email or send the form electronically to the patient via Portal and Concierge.

📝 Please note: All items on the patient's chart will be included on the estimate by default. You can edit the item settings on the chart if you didn't want a specific code to appear.

🇨🇦 Please note: If you are a Canadian or Australian practice, you can hide planned treatments and fees from your estimate templates at a site level.


Previewing an estimate before you share it

When you create or edit an estimate, a Preview panel shows how the finished document will look. The preview updates automatically as you change the template, practitioner, body text, footnote or payment terms, so you can check the layout and wording before the patient sees it.

  • If the preview is hidden, click 'Show preview' in the toolbar

  • Use 'Hide preview' if you need more space while editing the estimate body

  • A 'Template' must be selected before the preview will load

  • The preview matches the PDF and the emailed estimate

📝 Please note: Estimate preview is being rolled out to practices gradually. If you do not see a Preview panel or a 'Show preview' / 'Hide preview' button, it is not yet available for your practice.

🇦🇺 Please note: For Australian practices, the estimated health fund rebate only appears in the preview and on the PDF once the health fund estimate process has been completed by swiping the patient's health fund card. Read more about health fund estimates.


How to add a new estimate or consent form?

You can add or edit estimate and consent form templates from 'Settings'

  • Go to 'Templates'

  • Choose 'Estimates'

  • Both treatment estimates templates and consent templates are created and edited from this location.

  • Click 'New Template' button on the right-hand side.

From here you can add your content and create a new consent or estimate form, follow our guide here for details on creating new forms.

You can also edit existing forms from this location by choosing the desired form on the left and then editing the content inline before clicking 'Save'.


How to add Dentally Vision images to an estimate

You can include Dentally Vision images when creating or editing an estimate.

  1. After creating a new estimate select 'Add images'

  2. Choose one or more images to include

  3. Confirm your selection

The selected images will then appear under 'Attached images'. You can preview images in this section as well as removing or adding additional images as needed.

When images are attached, they will be included at the bottom of the generated estimate PDF.


How to print, email or send my form electronically to be signed

  • After clicking ‘Create’ on the estimate/consent form, a PDF preview of the form will open in your preferred PDF program.

  • From here you can Print the estimate for the patient through your PDF program view.

  • Once you close the tab to return to Dentally, you will see the option to 'Email' the estimate from the patients 'Chart' screen as well.

  • If your practice has estimate preview, you can check the Preview panel before clicking 'Create', and use 'View PDF' to open the PDF version at any time.

  • The form will also have been saved straight to the patient’s correspondence screen.

  • From here you can click on the form, to be able to print/email it straight from the patients correspondance.

  • The saved estimate will also automatically be available for the patient to sign electronically on their Portal or via the Concierge table app.


How to update the form after saving

  • Once you have saved your form using the 'Create' button and closed out of your PDF view, you can edit the form if needed from this screen.

  • If you have already navigated away from your 'Chart' you will need to generate a new form for the patient.

  • Make the changes on screen and click the 'Update Estimate' button.

  • This will save the changes and take you back to the view of the estimate in your preferred PDF program.


Creating multiple estimates

In some instances, you may need to create multiple Estimates or documents which require signing for the same treatment plan.

For example, you may with to create an Estimate and one or several Consent forms for the same course of treatment.

To do this:

  • Once you created your estimate

  • Click the 'Create New Estimate' button

  • The previous document will be saved to the patient's correspondence tab.

  • A new document will be opened for you to start again.

  • You can do this as many times as you need.
