πŸ“ Please note: To submit a claim the patient must have a CHI number or an attempted CHI search and the Exemption (if any) must be listed correctly before you can proceed.

  • Ensure that there are no outstanding Prior Approval requests for any of the treatment you wish to claim for.

  • Make sure all the items you wish to claim for are ticked as complete on the right hand side of the treatment plan and click on 'Submit claim' and then 'Submit dental claim'.

  • This will open up a preview of your claim with the items you are claiming for the charges associated. Please double check both the CHI number and Exemption at the top of this screen!

  • When you are sure the details are correct, click on 'Next'.

  • Once you are happy everything is correct click on 'Next'.

  • You will now be prompted with the declarations screen, read and tick these and click on 'Submit'.

  • You will be prompted to confirm you want to submit the claim, simply click 'Complete' to finish the submission process.
